Patient Resources

We want to make your experience at our office the best possible.  To assist us in taking care of your dental needs, we offer you the following resources when coming to our practice.  Please take some time to view some of our policies and learn more about our office.  Please feel free to call us if you have any questions about our practice, or if you would like to schedule an appointment.


We see all patients on an appointment basis and ask that you call in advance so we may reserve time for you.  The office phone number is (574) 825-1252.  We try to see all patients at their appointed time and request that you arrive on time for your appointments. If you arrive late, we may need to reschedule your appointment.

If you cannot keep an appointment, please call (574) 825-1252 to notify us immediately.  We request at least 24 hours notice if you must cancel an appointment so we may give this appointment time to another patient.  You may be charged for a broken appointment if you fail to give us 24 hours notice.

Based on OSHA recommendations, we no longer allow infants and small children in the treatment rooms while parents or other siblings have their dental work done, unless their presence is requested by a staff member or the dentist.  Other children and adults are to remain out of the treatment areas during family member’s dental care.  This includes appointments with the dental hygienist as well as the dentist.

We request that parents and guardians find outside childcare for the time they will spend in the dental office.  We request that they bring another adult to watch their young children in the reception area during their appointment or while their assistance is needed with another child’s treatment.  We thank you in advance for your cooperation in helping us keep your family’s treatment and our dental staff’s work areas safe.

Fees & Payments

We make every effort to keep the cost of your dental care affordable.  You will be given a treatment plan with an estimated cost.  Payment is expected at the time of service, financial arrangements (if needed) can be made with a member of our business office staff.  Patients may also qualify for private funding through Care Credit, which offers up to 12 months of interest-free payments, as well as extended duration interest-included payment options.


Dental Benefit Plans are employer and self-purchased insurance plans that are intended to cover a portion, but not all, of the cost of your dental care.  Dental plans may include a deductible and other non-covered service expenses which must be paid by the patient.

If you have a Dental Benefit Plan, please bring your card with you to each visit.  We will work with you to assure that you receive the maximum benefits to which you are entitled.

While we are a fee for service practice, we do file all insurance claims for our patients.  The dental insurance coverage is between you and your insurance company, and we can not be responsible for anything other than filing the claims.  The patient is ultimately responsible for their account.
